Your cousin doesn’t need to register the domain with cloudflare; she can use their dns service without doing so. With that said, registering the domain with cloudflare has the benefit of being extremely cheap and seamless (as long as your payment method works well with their gateway) as they do it at cost. The drawback of registering your domain and dns with cloudflare is that it’s a single point of failure. There are accounts of some people using cloudflare as their dns and registrar for contentious websites (illegal services, medical advice, etc.) whose websites got unexpectedly shut down by cloudflare.
When onboarding with cloudflare, they usually just guide you through setting your website up with their dns. This page should have all you need; in my experience cloudflare even pulls your existing records off your current nameserver.
Be prepared for a slight amount of downtime since you might need a few minutes to configure things correctly, but it should be pretty simple otherwise.
